Surgical Anatomy of Hernia Rings for Reduction of Incarcerated Hernias

Abstract
Acute incarceration or strangulation of various abdominal wall hernias is relatively common in surgical emergencies. Among majority of the patients, it can easily be reduced, but sometimes it is very difficult for patients with bowel wall edema and tight hernia ring. Traditionally, they have been managed with open surgeries . In recent years, however, laparoscopic surgeries have been attempted, but some controversies still persist. For patients whose hernia contents are difficult to reduce, the excessive force used might cause disasters of serosal or full-thickness tears of the bowel. Such tears, if unrecognized, can lead to postoperative leaks and peritonitis/sepsis together with needs for re-operation. In case of incarceration or strangulation, we, surgeons, must know the precise anatomy of the hernia ring and acquire the skills to extricate the bowel without further injuries to the nerves and vessels. In this article, the hernia rings of various abdominal wall hernias will be reviewed.
